How to Register

To register with our practice please complete our online registration form.

New Patient Registration Form- Adult
New Patient Registration Form – Child under 16

If you have not already registered with the National Health Service you will also need to complete a GMS1 which can be found here.

If you have your NHS number, please include this on the registration form. It does not matter if you do not have it but it may help us to trace your medical records more quickly.

At the time of registration we ask for ID for all adults aged over 16. One item should be a photo ID i.e. passport or driving licence along with one document containing your address, however please do not hold up applying for registration if you do not have access to these documents.

We recommend that you get one month’s worth of medication from your existing GP practice before submitting your registration as you will need to be seen by a member of the clinical team here before we can supply those medications. Please contact the Patient Services team to make an appointment. The Dispensary team will advise dispensing options available to you.

Summary Care Record (SCR)

Care professionals in England use an electronic record called the Summary Care Record (SCR). This can provide those involved in your care with faster secure access to key information from your GP record.

New Patient Health Check

All new patients aged over 16 are invited to make an appointment for a routine health check with a nurse or healthcare assistant. If you are taking regular medication or have any significant medical conditions we advise you to book an appointment with your new doctor.
